About Us

Alzheimer's Community Care (ACC) was founded in 1996 to provide community-based, dementia-specific services to patients and families living with Alzheimer's disease or related neurocognitive disorder. The Organization’s focus is on the wellbeing and safety of both the patient and the caregiver. ACC's dementia-specific care helps patients maintain quality of life after a diagnosis of Alzheimer's disease and gives caregivers the tools and education to care for their loved ones in the familiar surroundings of home. Our model of care is rooted in evidence-based, dementia-specific principles designed to keep patients in the community as long as possible, thereby avoiding or delaying costly nursing home placement. The model includes 11 Specialized Alzheimer's Day Centers, Family Navigators, 24-Hour Alzheimer's Assistance Line, ID Locator Services, Case Management, Professional Education, Training, and Advocacy. ACC is accredited by the Joint Commission and Nonprofits First and has the Platinum Seal of Transparency awarded by GuideStar.
